This is a Harbor Breeze 52 inch white ceiling fan.

The fan and lights are fully functional, but was removed to put a smaller fan over a bunk bed. It was installed in 2017. It has been disassembled in order to remove it safely, and has all screws and pieces included, but not the instructions for installation.
